Qwest® Home Phone is a package which includes a flat rated residential line and a choice of any or all of the features included in the Features Chart in the Ordering Section of this document.
Additional features not listed in the Ordering Section may be added to the package and will be billed at the monthly Tariffs/Catalogs/Price Lists rate, less any applicable Wholesale discount.
Qwest® Home Phone Package is available where facilities exist throughout Qwest's 14-state local service territory.
For specific feature compatibility and restrictions, see the individual feature Product Catalogs (PCATs).
Qwest® Home Phone Package can not be provided on the same line with other packages.
Qwest® Home Phone Package is compatible with residential line Universal Service Order Codes (USOCs) 1FR, AFH, HFR, AFL, RUA.
All features associated with the package must be on the same telephone number as the package USOC (PGOQW).
Qwest® Home Phone Package may have a recurring and a nonrecurring charge based on state specific Tariffs/Catalogs/Price Lists.
There is no charge to add or remove features on an existing package. There is no charge to add a package to an existing line or change an existing package to Qwest Home Phone.
Retail rates for this package can be found in the state specific Tariffs/Catalogs/Price Lists.
Rates and/or discounts are available in Exhibit A or the specific rate sheet in your Interconnection or Resale Agreement.

Line-Backer™ is an optional service which will be a promotional offering available at no charge for the first three (3) months when combined with the Qwest® Home Phone Package. After the initial 3 month period, if the service is retained, Line-Backer will be offered at a discounted monthly rate which can be found in Tariffs/Catalogs/Price Lists. If you choose not to retain Line-Backer, you must submit a request to remove the service.
